(Dân trí) – Bác sĩ cảnh báo, nếu dị vật gây tắc nghẽn hoàn toàn đường thở, trẻ sẽ có biểu hiện suy hô hấp, tím tái, ngưng tim và nguy hiểm tính mạng.
Vừa qua, Bệnh viện Nhi đồng 2 (TPHCM) đã tiếp nhận một trường hợp bệnh nhi 7 tuổi tử vong, nghi do nghẹt đường thở thương tâm.
Dị vật đường thở gây hậu quả nặng nề
Theo báo cáo nhanh, vào khoảng 10h ngày 28/12, bé K.D.M. (7 tuổi, ngụ tỉnh Đồng Nai) được mẹ mua bánh bông lan cho ăn sáng. Khoảng 3 phút sau khi đến nhà cô giáo chủ nhiệm lớp 2, bé có biểu hiện tím tái và tay ôm cổ.
Phát hiện sự việc, cô giáo đã sơ cứu và đưa bé vào một phòng khám trên địa bàn, sau đó chuyển tiếp đến Bệnh viện Nhi đồng Đồng Nai. Tại đây qua thăm khám, các bác sĩ xác định bé có tình trạng hôn mê, môi tím, hóc dị vật, đã ngưng thở, ngưng tim 30 phút.
Dù các bác sĩ tại đây tiến hành lấy dị vật, đặt nội khí quản rồi chuyển gấp bé lên Bệnh viện Nhi đồng 2 theo yêu cầu của người nhà, nhưng khi đến TPHCM, bệnh nhi đã tử vong ngoại viện, hoàn toàn không đáp ứng với các phương pháp hồi sức.
Trước sự việc trên, đã có nhiều trường hợp trẻ tử vong sau khi hóc dị vật. Vào năm 2017, một bé trai 5 tuổi tại TPHCM đã không qua khỏi sau khi bị một miếng thạch rau câu lọt vào đường thở, gây suy hô hấp cấp.
Câu hỏi đặt ra là làm sao để nhận diện dị vật đường thở ở trẻ, cũng như thao tác xử trí thế nào khi phát hiện sự cố?
Bác sĩ chuyên khoa 1 Đoàn Thị Thanh Hồng, khoa Hô hấp 1, Bệnh viện Nhi Đồng 2 cho biết, dị vật đường thở là một tình huống cấp cứu thường gặp ở trẻ em. Đặc biệt, tình huống này có nguy cơ đe dọa đến tính mạng bệnh nhi, do dị vật làm tắc nghẽn đường thở, dẫn đến thiếu oxy.
Dị vật gây hóc thường gặp ở đường thở là các loại hạt (hạt dưa, hạt hướng dương, hạt điều,…) hay một số loại thực phẩm, các mảnh đồ chơi nhỏ. Khi trẻ bị hóc dị vật thường biểu hiện bằng những cơn ho sặc sụa dữ dội, khó thở. Một số trẻ có biểu hiện hoảng loạn hoặc ra dấu hiệu bị nghẹn ở cổ (đối với trẻ lớn).
Nghiêm trọng hơn, nếu dị vật gây tắc nghẽn hoàn toàn đường thở, trẻ sẽ có biểu hiện suy hô hấp, tím tái, ngưng tim. Theo bác sĩ Hồng, hầu hết các trường hợp trẻ khi đưa đến bệnh viện sớm đều được cứu chữa thành công. Dù vậy, việc xử trí ngay khi phát hiện trẻ bị hóc dị vật rất cần được quan tâm.
Cách sơ cứu trẻ hóc dị vật
Để thực hiện đúng các bước ban đầu, hạn chế thao tác sai có thể gây nguy hiểm tính mạng, bác sĩ lưu ý phụ huynh cần bình tĩnh, thực hiện các thủ thuật dựa trên các nhóm tuổi cụ thể.
Theo đó, đối với trẻ dưới 2 tuổi, cần thực hiện thủ thuật vỗ lưng, ấn ngực:
– Đặt trẻ nằm sấp đầu thấp trên cánh tay trái, giữ chặt đầu và cổ trước bằng bàn tay trái. Dùng ngón trỏ và ngón giữa trái đẩy cằm trẻ lên cho cổ trẻ ưỡn tránh gập đường thở. Sau đó, dùng gót bàn tay phải vỗ 5 cái thật mạnh vào lưng trẻ (ở khoảng giữa hai bả vai).
– Nếu thấy trẻ còn khó thở, tím tái lật ngửa trẻ sang tay phải và dùng hai ngón tay trái ấn mạnh ở vùng 1/2 xương ức 5 cái.
– Tiếp tục luân phiên vỗ lưng và ấn ngực cho đến khi dị vật rơi khỏi đường thở hoặc thấy trẻ khóc được.
Đối với trẻ trên 2 tuổi, cần áp dụng thủ thuật Heimlich:
– Tư thế ngồi hoặc đứng phía sau người trẻ sao cho thuận tiện vòng 2 tay qua người trẻ.
– Bàn tay trái tạo thành nắm đấm, đặt ngay thượng vị, dưới mũi ức phía trước ngực và bàn tay phải ôm lấy nắm đấm.
– Ấn mạnh từ trước ra sau và từ dưới lên trên cho đến khi dị vật ra ngoài.
Sau khi thực hiện các thao tác này, dù trẻ đã nôn dị vật, phụ huynh vẫn nên đưa trẻ đến bệnh viện để bác sĩ thăm khám.
Bên cạnh đó, bác sĩ cũng khuyến cáo gia đình không nên để trẻ đùa giỡn, quấy khóc trong lúc ăn, để tránh gặp phải sự cố đáng tiếc.
